What days are you open?

The office is open Monday through Friday. We are open on an average of 2 Saturdays a month. Our Saturday office hours have seasonal variation; typically, more in the fall and winter and less in the spring and summer.


What are your office hours?

The office is open 7:00 AM to 3:00 PM, and is closed between noon and 1:00 PM daily.


What is your cancellation policy?

To avoid a $25 charge for missing an office visit, a 24 business hour notification is required.

Since more time is allotted for a vasectomy appointment, urodynamic study, transrectal ultrasound of the prostate, and prostiva missing one of these appointments will result in a $50 charge.


Messages left on the answering service after the office is closed and within 24 business hours is not considered adequate notification to avoid a missed appointment charge. Monday office appointments must be cancelled before 3:00 PM the Friday prior to the appointment to avoid this charge.

It is the responsibility of the patient to make a scheduled appointment. Our practice has an automated telephone reminder service, but it does not work in every type of phone messaging program. Failure of this service to remind you of an appointment is not an excuse to waive the fee for a missed appointment.


What Can I do to facilitate telephone calls?

When you call the office requesting information from a member of our office staff, please leave both daytime and nighttime phone numbers. Cell phone numbers and beepers are also appreciated as it may facilitate a timely return of your phone call.


What is the policy for prescription renewals?

There is a $25 charge for phoning in a prescription unrelated to a recent office visit or a recent surgery.


What is the policy for copying and faxing medical records?

A customary fee is charged for copying and or faxing medical records according to the number of pages. We suggest that when records are requested that the patient retain a copy to prevent repetitive charges.


What do I bring to my appointment?

Being prepared may save you time and avoid additional expenses. Please bring the following:

  • Copies of your medical records including x-rays, surgical reports, discharge summaries, pathology and lab reports.
  • When possible bring in the actual x-rays for personal review by the physician.
  • When possible bring the actual pathology (glass) slides for review.
  • Your medications or a list of names, dosages, and times taken. Many larger pharmacies will print such a list as a courtesy service.
  • All insurance cards and referral and authorization forms.
  • Your primary physician's name, address and phone number.

How do I get to your office?



From the South:
Take I-75 North to the Rochester Road exit (Exit 67). Turn right (north) on Rochester Road. Proceed north approximately eight miles on Rochester Road to University Drive (also called Walton Boulevard). Turn left (west) on University Drive (Walton Boulevard). The Boulevard Shoppe's will be on your right directly across the street from Crittenton Hospital Medical Center. Park in the rear and enter the medical offices from the back entrance.

From the North:
Take I-75 South to the University Road exit (Exit 79). Turn left (east) on University. Proceed on University one mile to Squirrel Road. Turn left on Squirrel Road. Go one-half mile to University Dr (also called Walton Boulevard) and turn right (east). Proceed on University Drive (Walton Boulevard) about four miles, until you pass Livernois Road. The Crittenton Hospital Medical Center will be on your right just past Livernois; the Boulevard Shoppe's are located directly across the street on your left. Park in the rear and enter the medical offices from the back entrance.

From the West:
Take M-59 East to Crooks Road exit. Turn left (north) on Crooks Road. Go two miles to Avon Road and turn right (east) on Avon Road. Proceed one mile and turn left (north) on Livernois. Go two miles to University Drive (also called Walton Boulevard) and turn right (east). Crittenton will be on your right, just past Livernois and the Boulevard Shoppe's will be on your left directly across the street. Park in the rear and enter the medical offices from the back entrance.

From the East:
Take M-59 west to the Rochester Road exit. Turn right (north) on Rochester Road. Proceed four miles on Rochester Road until you reach University Drive (also called Walton Boulevard). Turn left (west) on University Drive (Walton Boulevard). The Crittenton Hospital Medical Center will be on your left just past Helen Street; the Boulevard Shoppe's will be on your right directly across the street. Park in the rear and enter the medical offices from the back entrance.

From downtown Detroit:
Take I-75 north towards Flint. Exit on University Drive (exit 79). Keep right at the fork in the ramp. Merge onto University Drive, continue east one mile to Squirrel Road and make a left turn. Continue to East Walton Drive and turn right. E. Walton Drive will change its name to University Drive (also called Walton Boulevard). The Crittenton Hospital Medical Center is located just east of the intersection of University Drive and Livernois Road. The Boulevard Shoppe's are located directly across the street on your left. Park in the rear and enter the medical offices from the back entrance.
